STEP 1) Submit all the Original receipts, invoices and other medical payment documents such as – a stamp from the financial institution showing the name, and doctor’s prescription followed by his or her signature.
STEP 2) A Bank Account Number should be provided to receive the amount refunded. It should be attached along with a bank statement from the bank.
STEP 3) After submitting the Bank Account Number and other details, you need to submit the 13-digit National Identification Number of the Insured Person along with the original name and the address as specified in the medical documents.
